Accident and Medical Forms
If a player on your team sustains a significant injury (i.e. one requiring medical attention), please fill out the attached form and forward it to your VP (Girls, Boys, House League). Accident Report Form. Team managers should also maintain a file of medical forms for all players with general information, health care numbers and emergency contact information. This is a great guide put together by The Canadian Soccer Association and Thinkfirst. Thinkfirst Foundation is a Canadian organization educating athletes/kids, parents & coaches about brain injury and sport safety.
Concussion:
A couple of important points:
* You don't have to lose consciousness to have a concussion.
* No player should continue to play if a concussion has occurred (until further evaluated)
